What is endometrial scratching?

Endometrial scratching is an additional procedure performed prior to starting any medication to prepare for your IVF, ICSI, or frozen embryo treatment. The purpose of this procedure is to create a more fertile environment for embryos to grow into healthy fetuses. It’s generally timed so that the scratch can take place 17–22 days into your menstrual cycle.


During the procedure, your doctor will insert a sterile plastic catheter known as a pipille to scratch the endometrial lining, which triggers your body’s self-repair process and releases chemicals and hormones that make the uterus more receptive to embryo implanting. 


Endometrial scratching is an outpatient procedure and doesn’t require any general or local anaesthetics despite being more invasive than many other fertility-enhancing procedures. After the insertion of the catheter, the procedure takes less than a minute to complete. 


Although many studies have shown conflicting results, this procedure has been known to increase the live birth rate by 5% per IVF cycle, and boost pregnancy rates by up to 50%.

What are the risks associated with endometrial scratching?

After an endometrial scratch, many women report mild cramping—not unlike menstrual cramps—and some spotting/bleeding. This can be remedied through pain medication.

 

If you did have a cervix infection pre-procedure, endometrial scratching can increase the chance of the infection spreading to the uterus post-procedure. Our fertility specialists take every precaution to reduce the chances of infection, including giving you antibiotics.

How do I prepare for the scratching procedure?

There is no specific preparation required before the scratching procedure.

 

That said, as this is part of IVF treatment, you may be asked to continue your ovary-stimulation medication.


Your doctor may also suggest taking over-the-counter pain-relief medications to ease the discomfort associated with the procedure.

How long does the effect of endometrial scratching last?

In most cases, the scratches in your endometrial lining heal within one menstrual cycle. In some rare instances, it may take up to three months for the scratches to heal completely.

Is an endometrial scratch the same as an endometrial biopsy?

No. Although a small piece of tissue is removed during an endometrial scratch, this sample will not be submitted for further analysis unless we have reason to believe that an endometrial abnormality could be the reason for your fertility challenges.
